Facelift (Rhytidectomy)
womanWith aging, the soft tissues of the face and neck follow the course of gravity. Jowls form, the lines between the lips and cheek deepen, and necks begin to sag. A facelift is the process of restoring those tissues to a more pleasing position. A facelift should not change your appearance but rather refresh it. Our patients appear rejuvenated, rested, happier-but not “done”.

When is it time for a facelift?
If you find, when looking in the mirror, that you are pulling up your cheeks or neck with your hands and like what you see, then it may be time. The operation basically restores tissues to their previous, more youthful location. It is not about stretching or pulling skin to make you look different.

How is a facelift done?
Using incisions hidden in the natural creases along the ear, excess skin is lifted up, and the underlying layer of muscle is repositioned to reduce looseness in the neck and jowls. The skin is then laid back down, without stretching, and the excess skin trimmed. The procedure can be done under local anesthesia with some sedation, or with general anesthesia depending on patient choice.

Can it be combined with other procedures?
Frequently patients will have other procedures such as eyelid lift, browlift, or facial laser to treat lines around the lip and mouth.

What is the recovery like?
Usually bruising persists for a week or two, as well as some swelling. Obviously everyone is a little different. Stitches are removed at a week and two weeks. It is important to remember that although appearances can be startling earlier after surgery, it improves quickly. You should plan on 4-6 weeks of recovery prior to a major social occasion such as a wedding or class reunion.

Are there different types of facelifts?
The extent of the lift depends on how much sagging or extra tissue there is present. A person with minimal jowls can have a “minimal access” or “short scar” facelift, of which we do many. A person with a lot of neck skin will require more of an incision (extending behind the ear) to get a good result. We do not do “threadlifts”, as these have proven to be quite unsatisfactory and a waste of money. You will see many marketing campaigns and infomercial’s regarding various types of lifts. You will also, after an internet search, find many complaints and lawsuits against these companies. With your face, you should choose a highly qualified, board-certified plastic surgeon to advise you about your particular needs. We evaluate every technique, explore every method, and certainly have the surgical skill to do anything. We will explain the options, and help you decide.

What are the risks?
Infections and bleeding are unusual but can occur. The most serious risk with facial surgery is injury to a nerve of facial expression. This is very uncommon and usually resolves, but can be permanent. Visible scarring on facelift incisions is unusual.

Rhinoplasty (Nose Surgery)
rhinoplastyNoses come in all sizes and shapes. Unfortunately, some of these sizes and shapes aren’t a good fit for the person who owns the nose. Rhinoplasty is the process by which the structure of the nose is altered to bring the nose more into harmony with the rest of the face.

Some nasal conditions such as humps or a crooked shape occur as a result of trauma. Other features such as a bulbous tip, a long nose, or excessively broad nose are gifts from our parents.

The best candidates for rhinoplasty are those with fairly stable personalities and realistic expectations. A rhinoplasty will not cure depression, bad relationships, or an overall poor self image. The best candidates can be very specific about what they want different in their nose (please get rid of the hump) as opposed to non specific (I don’t know, I just have never liked my nose.) Be sure to tell the doctor about any breathing problems related to your nose, as rhinoplasty might have an effect on them. The procedure can be done under general anesthesia or under local anesthesia with sedation.

Eyelid Surgery (Blepharoplasty)
facePerhaps the most dramatic aspect of our appearance are the eyes. They convey emotion very acutely, and unfortunately aging eyes can convey things that aren’t appropriate to our moods at all. Droopy brows or lids can make us look tired, aged, or angry when we’re not. It seems silly that a few grams of skin in the wrong place can be such a big deal, but that is the reality.

Restoring the proper position of the brow, removing excess lid skin, or tightening skin with removal or a laser, perhaps even a filler, can be as dramatically rejuvenating as anything we do.

Removal of excess skin and fat from the upper and lower lids is called blepharoplasty. This takes away the heavy, tired look from the eyes and can also improve vision if the skin is obstructing the eye. In some cases, insurance coverage is available for this procedure. Incisions are hidden in the natural folds of the eyes and are rarely visible. Laser resurfacing is often a very useful addition to these procedures.

Forehead Lift (Browlift)

If the brows are sagging down over the eyelids, or have a sad appearance, elevating the brows can really lighten a person’s look. This procedure is usually done with an endoscope for milder cases, or an incision at the hairline when skin removal is required. Both tend to heal quickly and make an immediate impact. This is often combined with upper lid skin excision (blepharoplasty) to obtain the sharpest, cleanest result. These procedures are most commonly done with sedation and local anesthesia, so recovery from the anesthetic is minimal.

Ear Surgery (Otoplasty)

Most ear surgery is done for prominent ears-ears that stick out. This happens for two reasons-either the bowl of the ear is too big and too deep, or the outer fold of the ear doesn’t form properly. Sometimes both are present. Prominent ears can be corrected by recreating the outer fold of the ear, and reducing the size of the bowl of the ear.

How is it done?
Incisions are made, usually on the back of the ear, sometimes just inside of the bowl of the ear, where they are not typically visible. Stitches are used to recreate the cartilage folds, and also to hold the ear back against the head. Sometimes excess cartilage in the bowl (concha) is removed. This procedure can be done under either local anesthesia with sedation, or with general anesthesia.

How about recovery?
We will ask you to wear some surgical earmuffs for a week or so to protect the tender ears. You may remove them to shower the next day. The ears are swollen for a week to 10 days, and the swelling isn’t really totally gone for a few months, so don’t worry if your ears seem a little big. Exercise is OK if contact is avoided.

Scar Revision
facial scarScar revision is performed to reduce the appearance of scars caused by injury or previous surgery. Scars are by definition permanent, but surgery can narrow, fade and otherwise reduce the appearance of severe or unattractive scarring, which is especially helpful in areas of cosmetic importance such as the face and hands.

There are many surgical methods of scar revision, only some of which may be appropriate for a particular type of scar or its location. These include surgical excision, skin grafts, flap surgery and Z-plasty (repositioning a scar so that it aligns with the face and is less noticeable). The effectiveness of scar reduction depends on a number of factors, including the nature of the injury, your body’s healing mechanism, the size and depth of the wound, how much blood supplies the area and the thickness and color of your skin.

Laser Treatments

Laser Resurfacing
A laser is delivered through a computer controlled device that automatically scans the area of skin being treated. Physicians can control the ablation and coagulation depth for either light or deep peels. As we age, the effects of sun damage and environmental pollutants take their toll on our skin, causing it to wrinkle, develop sun spots and look tired. A Sciton skin resurfacing procedure can reverse these effects and restore a healthy look to the skin. There are two approaches to resurfacing: full field, and fractional. In full field resurfacing, the entire surface area of the skin is treated. A laser beam is scanned across the treatment area and precisely removes a layer of skin. The body will naturally replace the old, damaged skin that was removed, with a fresh, healthy layer. Treatments can range from shallow (which might be appropriate for eliminating fine lines, sun spots and tired-looking skin) to deep (which is often used to improve skin tone/texture and eliminate deep wrinkles, even in difficult areas like upper lip lines and crow’s feet) and acne scarring. In fractional resurfacing, the laser removes pinpoint columns of skin, leaving the surrounding tissue intact. This approach allows the laser to controllably ablate and coagulate tissue to variable depth, while the surrounding intact tissue promotes rapid healing.

BBL
A new study recently published in the Journal of Investigative Dermatology shows that Sciton BroadBand Light (BBL™) treatments can change the expression of genes associated with the aging process to more closely resemble young skin. The study supports the hypothesis that regulators of aging can be altered in human skin using BBL technology to provide a functional change, rather than just a cosmetic mimic of youthful appearance. BBL treatments promote the appearance of youthful skin. This study reveals that the appearance of younger skin may be due to the differences observed in gene expression patterns of aged skin becoming more like young skin. Treatments from the study were performed on the FDA-cleared Sciton Joule using the BBL module.

Acne Treatments
Acne is a disorder resulting from the action of hormones and other substances on the skin’s oil glands, sebaceous glands and hair follicles. Propionibacterium acnes (P. acnes) are the anaerobic bacterium that causes acne. Severe acne can lead to permanent scarring. The resurfacing laser creates a wound-healing process producing fibroblasts that generate new collagen, plumping the skin and correcting skin imperfections.

CAN FOREVER YOUNG BBL REALLY STOP THE CLOCK ON THE APPEARANCE OF AGING SKIN?

WHAT WILL HAPPEN DURING THE PROCEDURE?

Your eyes will be protected with safety shields or glasses. Typically, there is no need for a topical anesthetic, however your clinician may choose to use one on more sensitive areas.

You may feel a quick, warm sensation as the light is absorbed by the targeted areas. Your procedure may take a few minutes to half an hour, depending on the size of the area to be treated and the type of procedure performed.

HOW LONG WILL IT TAKE TO RECOVER?

This is a noninvasive and gentle procedure with virtually no downtime. In most cases, you are able to apply makeup, return to work and resume most of your activities immediately.

WHAT SHOULD I EXPECT AFTER THE TREATMENT?

You may experience some redness that should resolve within a few hours. For some pigmented lesions, you will see a darkening of the treated area followed by fading and flaking off at a later stage. Your treated skin will feel smoother, pores will be less noticeable, and sunspots or uneven pigmentation will fade. Results depend on your condition, the number of treatments, and the area treated. Your clinician will provide you with complete information about the post treatment care and results.

WHAT AFTERCARE PLAN DO I NEED?

It is important to follow your clinician’s treatment plan. You may be given skin care products and instructions on how to use them. You will be sensitive to ultraviolet light and you must avoid direct sun exposure until completely healed. Fully cover your treated skin or use a good sunscreen whenever you go outdoors.

Pigmented Lesions
BBL BroadBand Light technology will effectively treat sundamaged skin.. The light energy delivered by the BBL will gently be absorbed by the unwanted melanin responsible for pigmented lesions in the upper layers of the skin. Over the next few days the pigmented lesions will darken and fall off. A series of 4 treatments is recommended with each treatment approximately 4 weeks apart.  Face, Neck,Chest,Hands, arms and legs are all areas that can be treated.

Rosacea/Vascular Conditions
Laser or light energy is selectively absorbed by the oxyhemoglobin resulting in heating of the blood vessels. With the correct wavelengths and fluence, the vascular lesion will be heated to above 70 oC. At that temperature, blood vessels will break down and shrink which leads to a reduction of the vascular lesion.  The treatment is customized per patient.  This type of treatment would include Rosacea, some veins, and cherry angiomas.

Hair Removal
Laser or light energy is selectively absorbed by the melanin in hair follicles and converted to heat, diffusing to and injuring stem cells in the bulb and bulge. Effective hair reduction can only be achieved during the active growth phase and hair reduction is considered permanent when a significant amount of hair does not return for a period of time longer than the complete growth cycle. After a series of treatments, most hair is permanently removed. However, if hair does regrow it will be finer and lighter than before. Here are before and after pictures. We offer our hair removal treatments per area as a package of 5 treatments each.

LASER TREATMENTS – FREQUENTLY ASKED QUESTIONS

What results can I expect?
Halo is a complete skin rejuvenation treatment. You will see improvements in overall tone and texture in addition to significant removal of discoloration, reduction in fine lines*, reduced pore size, skin reflectivity and glow.
How many treatments do you need?
What is great about Halo is that you can choose how many treatments you want! Discuss with your clinician your needs and you can choose to have one or two more aggressive treatments or have multiple treatments that are less aggressive and achieve a similar goal over a longer period of time.
What happens during the treatment?
Thirty minutes to an hour before the treatment your physician may place a topical ointment on your skin to reduce the sensation during the treatment. Your clinician will then measure your face with motion tracking technology to ensure your face is treated evenly. The clinician will treat a section of your face at a time by rolling the Halo handpiece in a crisscross pattern until the system recognizes the treatment is complete.
What does Halo feel like?
With a topical anesthetic and integrated cooling technology, most patients experience very little discomfort. Most patients describe the treatment as a feeling of heat with occasional prickling sensations, but are generally comfortable. After the treatment, most patients will continue to feel warm for an additional 15 to 30 minutes.
Can I return to normal activities after my treatment?
Absolutely. For the first 24 hours you should keep your skin protected according to your clinician’s instructions, but after that you should be able to go about your normal activities. After the first 24 hours your skin will begin to peel, but you will be able to put on makeup. For a facial treatment, you will see very little change on the first day. After two to three days you will develop a light bronzing appearance. By day three or four your skin will peel, revealing your new skin beneath.
When will I see results?
You will see initial results between two and five days after your Halo treatment, but your skin will continue to improve over time. Most patients see the greatest pigmentary improvement in the first two to three weeks. This is followed by a dermal regeneration phase that continues to improve for months.
How long will my results last?
Your Halo results will last for years to come, but you should continue to keep your skin protected at all times using UV sunscreen to protect your new, healthy skin. Each additional Halo treatment you get will only improve the results, giving you even better looking skin.

Micro needling

Billings Plastic Surgery features seasonally-based facials.  For Fall, we’re proud to offer a Ginger Vanilla Enzyme facial.  All of our facials can be customized to treat skin types such as dry or oily.  Our facials can also be added on as an extra treatment to some of the services we offer. All of our facials include we use natural fruit enzymes, antioxidants, natural brighteners, and exfoliants  to treat acne, melasma, pigment, rosacea, and dry or oily skin.

 

DermaPlaning
Dermaplaning is a method of exfoliation that consists of using a 10 gauge scalpel to gently scrape off the top layer of dulling dead skin cells in order to reveal a smoother, brighter complexion. Dermaplaning is a favorite in the entertainment industry, specifically among models and actresses. Why? It also eliminates that annoying peach fuzz that cakes up in makeup and can be enhanced by harsh production lighting and high-definition cameras. “Dermaplaning removes all the dead skin cells and also the facial hair, leaving the surface very smooth.    Dermaplaning also allows for greater penetration of skincare products and creates a flawless canvas for makeup to glide on smoothly. In fact, cosmetic dermatologists often use dermaplaning to help prepare skin for more intrusive procedures such as laser treatments and chemical peels.

 

WHO IS IT FOR?

Dermaplaning is recommended for all skin types except those with acneic skin. If you have deep cystic acne, wait until breakouts are clear before trying this treatment. As always, also discuss any allergies or skin sensitivities before starting a treatment.

This is a great treatment if you are pregnant or nursing and want exfoliation without the risk of harsh chemicals absorbing into the blood stream and potentially harming your baby.

WHAT DOES THE TREATMENT FEEL LIKE?

Dermaplaning is a painless procedure and can be compared to the sensation of shaving your legs, but in this case, it’s on your face.

First, the aesthetician will cleanse and thoroughly dry your skin. Next, she’ll pull an area of your skin taut with one hand, and hold a sterile 10 gauge blade at a 45 degree angle in the other hand and use short, swift strokes to scrape off dead skin cells and peach fuzz.

HOW LONG DOES IT TAKE?

About 30 minutes

ARE THERE IMMEDIATE SIDE EFFECTS?

There are no side effects and zero downtime. After the treatment, your skin will be highly sensitive to sun exposure and may already look very pink, so be sure to wear a broad-spectrum sunscreen with SPF 30 or higher to reduce the risk of hyperpigmentation.

WHAT ARE ANTICIPATED RESULTS?

You’ll immediately look more radiant with a more even skin tone and texture. All of your peach fuzz will be gone, which is why first-timers say their skin has never felt so soft before.

RECOMMENDED COURSE FOR OPTIMAL RESULTS?

Dermaplaning removes 21 days worth of dead skin cells, so it’s best to allow the skin to complete its rejuvenation cycle before your next treatment approximately 6-8 weeks.

WHO SHOULD GIVE ME THIS TREATMENT?

Dermaplaning should be administered by a board certified dermatologist or licensed aesthetician who specializes in this procedure. “An aesthetician has to be licensed in the state she is practicing in. There are several different schools you can attend, but ultimately you have to pass a state administered written and practical exam to get your license,” explains Wright. It’s always a good idea to ask how long they’ve been administering dermaplaning to know their level of experience—there’s a very sharp blade involved!

TCA Cross Peel
The TCA Cross Peel does not treat the full face.  The TCA is placed in specific area of acne scarring.  The peel may need to be repeated 2-3 times and the results can be impressive.  The TCA Cross Peel can also be given before a pro fractional or resurfacing laser to improve acne scarring.

Vollure

Allergan, the pharmaceutical company that owns Juvéderm, just announced its newest hyaluronic acid dermal filler, Juvéderm Vollure XC, which has been approved for the correction of “moderate to severe facial wrinkles and folds, such as nasolabial folds,” according to a statement made by the company. What’s more is that this filler, made for those over 21, is said to last as long as 18 months — a full year longer than other injectables on the market.

In a clinical trial for the injectable, 59 percent of participants saw improvement in smile lines around the mouth for up to 18 months, and at the same time, 82 percent of patients said they were satisfied with results at six months, while 68 percent were still pleased at eight months .

Juvéderm Vollure XC uses special technology, known as Vycross — which is also used in Juvéderm Voluma XC and Juvéderm Volbella XC — to hold the hyaluronic acid together into a cohesive gel, which keeps it intact after it’s injected into the skin. “Vollure XC’s Vycross technology links both high and low molecular-weight hyaluronic acid, while Juvederm’s Hylacross technology uses various degrees of cross-linking of mainly high molecular weight hyaluronic acid.” Another major difference between Juvéderm and the new Juvéderm Vollure XC is that the new formula produces minimal swelling, which means shorter recovery time.
